Eziplan can provide you with an electronic Incident Reporting System linked into your Care Management. (If you do not have access to Incident Reporting, please contact your Ezihub Support Team through the Help Desk)

Incidents can be created both in the Eziplan Admin Portal by going to the 'Incidents/Hazards' option under the Client/Participant tab in the Left Hand Minimisable Sidebar 

And then choosing the "Add New Incident" button.

or via the Mobile App by opening the 'Incidents' tab on the Home Page.
